Admission Registered Nurse (RN) - HospicePay: $50 - $65 per hour

JOB DESCRIPTIONWe are seeking a compassionate and experienced
Hospice Admission Registered Nurse (RN) to conduct comprehensive patient assessments, develop individualized care plans, and coordinate hospice services for patients in their homes. The Admissions RN plays a critical role in ensuring a smooth transition into hospice care while providing education, support, and guidance to patients and families.
Key Responsibilities- Complete comprehensive hospice admission assessments, including physical, psychosocial, and medical history evaluations.
- Develop and initiate individualized hospice care plans based on patient needs and goals.
- Provide skilled nursing care, symptom management, and patient education.
- Collaborate with physicians, interdisciplinary teams, caregivers, and community resources to coordinate care.
- Educate patients and families on disease progression, medications, comfort care, and self-care techniques.
- Monitor and document patient status, interventions, and outcomes in a timely and accurate manner.
- Communicate changes in condition and obtain physician orders as needed.
Qualifications- Current and unrestricted California Registered Nurse (RN) License.
- Minimum of 2 years of RN experience; hospice, home health, palliative care, or public health experience preferred.
- Strong clinical assessment, care planning, and communication skills.
- Current CPR/BLS Certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- Ability to travel to patient homes and perform physical job duties, including standing, bending, lifting, and transferring patients as needed.
Pay Range: $50 - $65 per hour
