Job SummarySeaport Scripps Home Health is seeking an experienced
OASIS Review Nurse (Registered Nurse - RN) to support clinical quality, regulatory compliance, and documentation integrity in a
home health setting. This role is responsible for reviewing and ensuring the accuracy, completeness, and consistency of
OASIS assessments and
ICD-10 coding, while collaborating closely with clinical and leadership teams to improve outcomes and agency performance.
This position is ideal for a
detail-oriented Home Health RN with strong knowledge of
OASIS, Medicare regulations, quality assurance, and clinical documentation review.
Key Responsibilities- Prospectively review all OASIS assessments to ensure accuracy, completeness, and compliance with federal and state home health regulations and Seaport Scripps Home Health standards
- Analyze OASIS variation and alert reports to identify documentation gaps and trends
- Ensure correct ICD-10 coding and sequencing, including primary diagnoses and co-morbidities
- Collaborate with home health clinicians to clarify documentation issues and support appropriate corrections
- Review visit utilization to ensure services align with patient condition, care guidelines, and reimbursement requirements
- Identify potential underutilization, overutilization, or financial risk and report findings to Clinical Supervisor or designee
- Communicate problematic trends in OASIS data, outcomes, and processes to leadership
- Participate in Quality Improvement (QI), performance improvement, and Corporate Compliance activities
- Assist with additional chart audits and clinical documentation reviews as needed
Qualifications & Requirements- Registered Nurse (RN) with an active California license
- 2+ years of home health nursing experience required
- Strong working knowledge of OASIS (OASIS-E preferred) and ICD-10 home health coding
- In-depth understanding of CMS regulations, Medicare Conditions of Participation (CoPs), and state licensure requirements
- Strong analytical skills with attention to detail and excellent clinical judgment
- Effective communication and collaboration skills with interdisciplinary teams
