We are seeking an
Acute Care Nurse Practitioner (AGACNP) to join our collaborative nephrology team. Whether you are an experienced nephrology provider or looking to expand your expertise into a highly specialized field, we offer comprehensive training and mentorship to help you succeed.
Bilingual Spanish-speaking candidates are strongly encouraged to apply.What You'll Do- Evaluate, diagnose, and manage patients with acute and chronic kidney disease
- Collaborate closely with nephrologists and interdisciplinary care teams
- Develop and implement treatment plans to improve patient outcomes
- Provide patient education regarding kidney health, treatment options, and disease management
- Document patient encounters accurately and efficiently within the EMR
- Participate in care coordination across outpatient and inpatient settings
- Manage a small subset of dialysis patients as part of the practice's comprehensive care model
Schedule- Monday-Friday
- Daytime schedule
- Primarily office and inpatient-based care
- No routine weekend coverage
Qualifications- Master's or Doctoral degree in Nursing from an accredited program
- Current Acute Care Nurse Practitioner certification (AGACNP)
- Active Texas Nurse Practitioner license or eligibility to obtain
- Minimum of one year of Nurse Practitioner experience
- Strong clinical assessment, diagnostic, and care management skills
- Excellent communication and collaboration abilities
- Commitment to delivering high-quality, patient-centered care
- Nephrology or dialysis experience preferred, but not required
- Bilingual Spanish/English preferred
