Registered Nurse (RN) - Wound Care

Location: Onsite - Umpqua Valley Nursing & Rehabilitation | Roseburg, OR
Schedule: Full-Time | 37.5 Hours/Week | Monday-Friday
Pay: $52.00-$60.00 per hour (DOE)

The Wound Care RN is responsible for coordinating and overseeing the facility's wound care and skin management program. This role partners with the interdisciplinary team to assess residents, develop individualized treatment plans, monitor healing progress, educate staff, and ensure compliance with clinical best practices and regulatory standards. What You'll Do

  • Perform comprehensive skin and wound assessments for residents.
  • Develop, implement, and update individualized wound treatment plans.
  • Monitor wound healing progress and adjust interventions as appropriate.
  • Collaborate with physicians, therapy staff, dietitians, and the interdisciplinary care team.
  • Lead and support the facility's skin integrity and pressure injury prevention program.
  • Ensure accurate, timely, and compliant clinical documentation.
  • Provide education and guidance to nursing staff on wound care best practices.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Monitor infection prevention practices related to wound management.
  • Serve as a clinical resource for staff, residents, and families.
  • Maintain compliance with all federal, state, and facility regulations.
  • Perform other nursing duties as assigned.


Qualifications

Required

  • Current, active Registered Nurse (RN) license in the State of Oregon.
  • Strong assessment, documentation, and clinical decision-making skills.
  • Knowledge of infection prevention and skin integrity management.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively with residents, families, physicians, and interdisciplinary teams.
  • Passion for providing exceptional care to seniors in a skilled nursing environment.

Preferred

  • Previous wound care experience in a skilled nursing, rehabilitation, long-term care, or post-acute setting.
  • Wound Care Certification (WCC, WOCN, or equivalent) preferred-or willingness to obtain certification with employer support.
  • Experience with pressure injury prevention and treatment protocols.
  • Experience in electronic medical records (EMR) documentation.
