Qualifications

• Current, unencumbered state license as a registered nurse.
• Board certification in Wound Ostomy, Continence Nursing or completion of advanced practice nurse program.
• Three years of clinical wound care nursing experience, with one year in home health care preferred.
• Knowledge of home health care regulations and reimbursement models is preferred.
• Experience using virtual technology for remote consultations is a plus.
• Strong communication and collaboration skills are essential.

Responsibilities

• Evaluate assessments, documentation, and care plans for evidence-based treatment compliance.
• Provide virtual consultations for complex patients to enhance care quality.
• Conduct remote reviews of charts and wound images for high-risk patients.
• Recommend evidence-based wound and ostomy interventions tailored to patients' needs.
• Coordinate care to prevent and manage complications effectively.
• Utilize clinical judgment and advanced experience to create individualized care plans.
• Serve as a clinical resource for care center staff and clinicians on best practices.
• Collaborate with internal and external providers to optimize clinical care and outcomes.
• Stay updated on advancements and best practices in wound care.
