Wound Care CoordinatorWe are seeking a compassionate and dependable Wound Care Director to join our team and play a key role in ensuring the health, comfort, and dignity of our residents. As a Wound Care Director, you will provide hands-on care to residents, identify changes in condition, and evaluate care needs. You will also monitor overall condition and provide care in compliance with facility policies and procedures.
Responsibilities:- Assess, plan, and implement individualized wound care treatment plans in accordance with established standards of care
- Conduct wound inspections and assess wound status, documenting findings and progress
- Develop and implement treatment plans, including wound cleansing, dressing changes, and other interventions as needed
- Educate patients and family members on wound care and prevention strategies
- Collaborate with interdisciplinary healthcare team to ensure comprehensive care
- Participate in quality improvement initiatives to enhance wound care outcomes
- Maintain accurate and confidential patient records
- Provide hands-on care to residents based on their Plan of Care.
- Identify changes in resident condition and evaluate care needs.
- Demonstrate strong assessment and documentation skills.
- Evaluate and participate in the development of the Resident Plan of Care.
- Monitor overall condition and provide care in compliance with facility policies and procedures.
- Provide primary skin care to residents under the medical direction of the attending physician or Director of Nursing.
- Identify, manage, and treat skin disorders, lesions, and decubitus ulcers
Requirements:- Current and valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state.
- At least 2 years of clinical experience in a long-term care, rehab, or skilled nursing setting.
- Prior experience providing wound care in a post-acute or long-term care facility.
