URGENT HIRING: VA Clinical Manager - Palm Beach FL

Activa Home Health is seeking an experienced and highly motivated VA Clinical Manager to join our team. The ideal candidate will have strong knowledge of the Department of Veterans Affairs home health processes and proven experience managing VA-authorized episodes of care.

This is a key clinical leadership role responsible for overseeing clinical operations, ensuring appropriate episode management, supporting clinical staff, and maintaining compliance with VA requirements and home health standards.
Key Responsibilities
  • Manage and oversee clinical operations for VA patients receiving home health services.
  • Manage VA authorizations, including reviewing, tracking, and ensuring services are provided within authorized parameters.
  • Oversee episode management, including start of care, recertifications, resumption of care, and episode transitions.
  • Ensure accurate and timely documentation in accordance with VA requirements and home health regulations.
  • Monitor patient care plans and ensure services are delivered according to physician orders and authorized services.
  • Collaborate with clinicians, physicians, referral sources, VA representatives, and internal leadership to coordinate patient care.
  • Review clinical documentation for accuracy, completeness, and compliance.
  • Monitor patient outcomes, quality measures, and overall clinical performance.
  • Provide clinical guidance and support to field clinicians and other members of the care team.
  • Assist with resolving authorization, scheduling, documentation, and patient-care issues.
  • Maintain working knowledge of VA processes, requirements, and expectations related to home health services.
  • Ensure compliance with applicable federal, state, VA, and home health regulations.
  • Utilize Kinnser/WellSky for clinical documentation, scheduling, episode management, and related operational functions.
Required Qualifications
  • Current Registered Nurse (RN) license in the applicable state.
  • Previous experience as a Clinical Manager in home health.
  • Strong experience working with VA authorizations and VA home health processes is required.
  • Demonstrated experience with episode management in a home health setting.
  • Working knowledge of VA requirements, workflows, authorizations, and processes.
  • Kinnser/WellSky experience required.
  • Strong understanding of Medicare/home health clinical operations and documentation.
  • Excellent organizational, communication, and problem-solving skills.
  • Ability to manage multiple priorities in a fast-paced home health environment.
  • Strong leadership skills with the ability to support and hold clinical staff accountable.
Preferred Qualifications
  • Previous experience managing a VA-focused home health caseload or program.
  • Experience working directly with VA referral sources and/or VA representatives.
  • Experience with OASIS and home health regulatory requirements.
  • Previous supervisory or clinical leadership experience.
