Triage Nurse (In-Person)

Overview:
MFM Health is seeking a highly skilled and compassionate Triage Nurse to join our clinical team to support a rotating coverage in our Danvers, Middleton and Manchester-By-The-Sea practice locations This is an in-person full time position responsible for assessing and evaluating patients' reported symptoms, determining the most appropriate course of action, and ensuring comprehensive coordinated care. The ideal candidate is a team player with strong communication skills, clinical expertise, and the ability to handle a variety of situations with professionalism and empathy.

Key Responsibilities:
  • Patient Assessment & Triage:
    • Evaluate and assess patients' reported symptoms via telephone and in-person encounters.
    • Determine appropriate clinical response and urgency of care, including when EMS activation is warranted.
    • Administer in-person clinical triage and basic patient care as needed.
  • Care Coordination:
    • Communicate and collaborate with providers and clinical teams to ensure appropriate, timely, and comprehensive patient care.
    • Coordinate follow-up care, referrals, and ongoing treatment planning.
    • Review diagnostic results, referrals, and provider feedback with patients.
  • Communication:
    • Proficiently manage telephone triage, effectively addressing a wide variety of patient concerns.
    • Handle challenging or escalated patient situations with professionalism and composure.
    • Clearly and accurately document all interactions and care plans in the medical record.
  • Nurse Visits:
    • Review vaccine schedules and administer vaccines as indicated for adult and pediatric patients
    • Provide injection teaching as needed
    • Apply Zio heart monitors
    • Perform Pulmonary Function Testing, Diabetic Eye Exams
  • Teamwork & Support:
    • Work closely with other departments and assist clinical staff to meet the needs of the patient.
    • Actively participate in team communication and cross-functional support.
    • Serve as a resource by identifying teaching moments and constructively advising peers.
  • Professional Conduct:
    • Maintain a professional demeanor at all times.
    • Report concerns or critical observations to appropriate team leads or supervisors.
    • Adhere to organizational policies and demonstrate commitment to high-quality patient care.
