Qualifications

• Active or eligible Wisconsin Physical Therapist Assistant / PTA license.
• Prior experience preferred in skilled nursing, post-acute rehab, LTC, or geriatric settings.
• Preferred travel experience, adaptable for the right candidate.
• Familiarity with Net Health system is a plus.
• Strong documentation, communication, and team collaboration skills.
• Ability to start quickly and complete onboarding before assignment begins.

Responsibilities

• Provide skilled PTA services under licensed PT supervision.
• Implement treatment plans to enhance patient mobility and independence.
• Accurately document treatment sessions according to facility standards.
• Collaborate with PTs, rehab leadership, nursing staff, and care partners.
• Promote patient-centered care and assist in safe discharge planning.
• Demonstrate professionalism, flexibility, and teamwork in approach.
