Advanced Practice Provider-Outpatient Transplant (Full-time)

Piedmont Healthcare is seeking a Nurse Practitioner or Physician Assistant to join our Transplant Outpatient Team. This role is responsible for the ongoing management and coordination of care for liver transplant patients in a collaborative, multidisciplinary setting.

The ideal candidate is clinically strong, highly organized, and comfortable practicing with a high degree of autonomy while managing complex transplant patients.

  1. Provide comprehensive evaluation and management of liver transplant recipients.
  2. Develop and implement individualized treatment plans based on clinical assessment, laboratory, and diagnostic findings.
  3. Monitor graft function, immunosuppressive therapies, and post-transplant complications.
  4. Educate patients and families regarding transplant care, medications, and lifestyle modifications.
  5. Coordinate care with transplant surgeons, hepatologists, coordinators, pharmacists, and other members of the multidisciplinary team.
  6. Perform minor procedures within scope of practice.
  7. Obtain informed consents as appropriate.
  8. Participate in transplant program quality initiatives, research activities, and multidisciplinary conferences.
  9. Serve as a clinical resource for staff and support educational initiatives.
  10. Participate in after-hours call coverage approximately once per month, including holidays.


Position Details:

Outpatient, non-OR role

Full-time: 40 hours a week (Monday-Friday), 10 hour shifts 4 days a week

Minimum Education Required:

PA: Graduate of a Physician Assistants program with current license for the Georgia State Composite Board

OR

NP: Graduate from a school of nursing with current Georgia license for Advanced Practice Nursing and a Master's degree in Nursing
