Job Summary and ResponsibilitiesAs an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.
Every day, you will expertly manage the full spectrum of transplant coordination: from initiating referrals and evaluating candidates, to leading in-hospital patient care, managing education, facilitating communication with all stakeholders including payers, and maintaining critical data.

- Organizes the clinical program
- Develops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient population
- Develops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.
- Serves as resource to hospital staff regarding patient care.
- Coordinates the patient evaluation process.
- Performs initial assessment of patient and family according to protocol for acceptance into program
Job RequirementsMINIMUM- Graduate of accredited school of nursing
- Three years experience with patients in a related setting or three yearsexperience in an intensive care, acute care, telemetry or medical/ surgical setting
- Registered Nurse: AZ
PREFERRED- Bachelors of Nursing or higher degree
- Critical care experience
- Program Management/Development experience
- Care Coordination experience
