Transition Coordinator Job Description

The Transition Coordinator is responsible for overseeing, expanding, and aligning Bridgeport Adult Education's comprehensive transition services to ensure all exiting students complete a documented Career Readiness Portfolio aligned to identified career pathways.

MINIMUM QUALIFICATIONS:

  1. Bachelor's degree required
  2. Minimum 3-5 years of experience in adult education, workforce development, or career services preferred
  3. Strong data, organizational, and partnership-building skills required.

MAJOR AREAS OF RESPONSIBILITY

  1. Lead the development and implementation of the Adult Education Transition Plan.
  2. Oversee and the Career Development Center.
  3. Supervise and support Workforce Developers.
  4. Develop and implement career pathways in Healthcare, Manufacturing, and Education.
  5. Coordinate job shadowing and internship programs.
  6. Facilitate partnerships with workforce boards, American Job Center, employers, and training providers.
  7. Facilitate quarterly partner meetings
  8. Track and analyze student transition data and prepare quarterly reports.


Student Outcomes and Accountability

Ensure 90% of exiting students complete a Career Readiness Portfolio, including career pathway identification, goal-setting meetings, CTHires enrollment, digital literacy completion, resume development, and a post-graduation plan.

Professional Learning and Staff Collaboration

Provide orientation and ongoing support to instructional and counseling staff on transition practices, CCRS/ELPS alignment, UDL, cultural competency, and trauma-informed approaches.

Performance Measures

Success will be measured by portfolio completion rates, career pathway participation, job shadow/internship placements, quarterly reporting, and improved student retention outcomes.
