The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

The Supervisor, Social Worker, in collaboration with the interdisciplinary care team, provides social work clinical services, including discharge planning, to individuals and families of all ages and is responsible for the recruitment, training, scheduling and supervision of the ED social work staff. Hours may include days, evenings, weekends and overnights. The senior social worker, in conjunction with the Manager of Emergency Department Social Work Services, ensures optimal psychosocial care to patients seen in the emergency department.

ESSENTIAL FUNCTIONS
  • Provides social work interventions to patients and their families including emotional support in crisis situations, information about and referral to community resources, facilitation of patient transfer to other care facilities and other services relevant to the patient's discharge from the ED; Performs psychosocial assessments of patients identified through referral and/or risk assessment
  • Responsible for recruitment, training, scheduling and supervision for ED social work staff; provides onsite supervision opportunities during all shifts.
  • Conducts annual performance evaluations and competency evaluations on assigned social work staff, and monitors their professional practice;
  • Participates in after hours on call schedule
  • Ensures that Hospital and Social Work Division policies and procedures are implemented and adhered to by the assigned social work staff with regard to professional practice, patient care and discharge planning
  • Collaborates with ED physicians, nursing and Community Health Nursing staff in developing an interdisciplinary care and/or discharge plan for the patient
  • Provides appropriate assessment and intervention services for children and adults suspected of being victims of abuse, violence, neglect and/or domestic violence;
  • Other duties as assigned


MINIMUM EDUCATION & EXPERIENCE
  • Master's degree from an accredited school of Social Work and 2 years of professional social work practice required
  • Or equivalent combination of education and experience


LICENSES AND CERTIFICATIONS
  • LMSW - Licensed Medical Social Worker - State Licensure as defined by the State Board for Social Work, State Education department, Division of Professional Licensing Services upon hire required
