Job Summary and QualificationsThe Registered Nurse (RN) Case Management Supervisor is a working leader who promotes patient-centered care by coordinating the plan of care throughout the patient stay, managing length of stay, ensuring appropriate resource utilization, and developing a safe and appropriate discharge plan in collaboration with the multidisciplinary team. The Supervisor facilitates the progression and transition of care using established criteria and supports quality outcomes and patient throughput while balancing optimal care and resource efficiency.
In addition to maintaining a full caseload, the RN Case Management Supervisor provides clinical leadership and operational support to the RN Case Management team. This includes mentoring staff, monitoring documentation and regulatory compliance, supporting daily operations, and collaborating with the Manager to implement performance improvement initiatives and staff development programs. The Supervisor serves as a resource for complex cases, escalates barriers to care, and fosters a culture of accountability, excellence, and teamwork
Clinical Case Management Duties:- Coordinate patient care plans and facilitate safe, timely transitions across care settings.
- Conduct comprehensive assessments of medical, psychosocial, and discharge needs.
- Collaborate with physicians, nursing, and interdisciplinary teams to optimize patient outcomes.
- Utilize evidence-based criteria (e.g., InterQual) to evaluate progression of care.
- Document case management activities, discharge planning, and care coordination in the EMR.
- Identify patients at risk for adverse outcomes and implement mitigation strategies.
- Partner with Social Services to address psychosocial needs and ensure appropriate post-acute care.
- Educate patients and families on healthcare resources and discharge options.
- Monitor length of stay and escalate avoidable delays through appropriate channels.
Supervisory Duties:- Provide daily guidance, coaching, and support to RN Case Managers.
- Assist with onboarding, training, and competency validation for new and existing staff.
- Monitor quality of documentation and adherence to CMS, TJC, and hospital standards.
- Participate in staff evaluations and provide input on performance and development.
- Support scheduling, coverage planning, and workload balancing.
- Track and report departmental metrics, including choice letters and discharge appeals.
- Collaborate with leadership on performance improvement initiatives and action plans.
- Serve as a liaison between frontline staff and leadership, promoting a culture of accountability and excellence.
- Actively participate in multidisciplinary rounds and department meetings.
What qualifications you will need:Education & Experience:
- Associate Degree in Nursing required; BSN preferred Required
- Minimum 3 years clinical nursing experience. Required
- Minimum 2 years case management experience. Required
- Prior Team Lead or Supervisory experience preferred Preferred
Licensure, Certifications, Training:
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation. Required
- Certification in Case Management Preferred
- Certification in InterQual or IQCI Preferred
