Sub- Acute Resident Care Manager - Registered Nurse (RN)

Location: Medford Leas, Medford, NJ

Salary: $74,900 - $97,000 per year
Schedule: Full-time | On-site | Mon-Fri 8am - 4:30pm

Position Overview

The Resident Care Manager (RCM) plays a key leadership role in ensuring the delivery of the highest quality, resident-centered care. The RCM promotes resident independence, dignity, and well-being through clinical excellence, effective team leadership, and interdisciplinary collaboration.
This position oversees a 14-bed sub-acute rehabilitation unit, providing hands-on guidance to nursing staff and ensuring regulatory compliance and clinical best practices.

Key Responsibilities
  • Maintain 24-hour accountability for unit operations and participate in the on-call rotation
  • Assess, monitor, and coordinate individualized care plans for residents.
  • Supervise, mentor, and evaluate nursing staff performance.
  • Ensure compliance with all state and federal regulations and uphold resident rights.
  • Collaborate with physicians, interdisciplinary teams, and families to optimize care outcomes.
  • Participate in quality improvement, infection control, and staff development initiatives.
  • Collaborates with RNAC/MDS coordinator to establish individual care plans based on the Care Area Assessment triggers from the Minimum Data Set ( MDS)

Requirements

Qualifications:
