  • Review patient records for compliance with The Joint Commission Core Measures.
  • Conduct post-acute stroke discharge follow-up calls to address care coordination needs.
  • Coordinate community education and stroke support groups.

Job Summary and Responsibilities

As our Stroke Nurse, you will demonstrate mastery of the specific knowledge and skills required for a specialty in nursing. Under the direction of the Director of Neuroscience Operations, you will be responsible for facilitating and supporting care for patients who are suspected or confirmed of having an acute stroke.Every day, you will apply your knowledge of standards and evidence-based practice to enhance the quality of care for both the patient and their family. Your primary function will be assisting with the care of patients during the hyperacute phase of stroke. You will serve as the crucial clinical resource in providing direct and indirect care, aimed at optimizing patient assessment, continuity of care, education, and the facilitation of ongoing treatment. You will also serve as a key contact for physicians, referring physicians, hospitals, patients, and families to facilitate the collaboration and coordination of patient care services, ensuring the highest level of care possible. To be successful in this role, you will possess exceptional clinical expertise in acute stroke care, strong communication and coordination skills, and a deep understanding of evidence-based practices and regulatory compliance. Your ability to effectively manage care during the hyperacute phase, serve as a vital clinical resource, and ensure meticulous documentation of quality initiatives will be essential in improving stroke patient outcomes and maintaining high standards of care.

 

  • Conducts daily rounding on all assigned neuroscience program patients, with multiple daily rounds in critical care units.
  • Conducts daily physical assessment on all assigned neuroscience program patients, as appropriate. Assists with immediate detection of any neurological or other medical status change and provides immediate intervention to patients in distress within scope of practice. Documents assessment in medical record and writes appropriate orders based on assessment, within scope of practice and in collaboration with supervising physician. Performs patient history and
  • physical as requested by MD.
  • Determines eligibility of stroke patients for enrollment in the program
  • Assesses patients' clinical and educational needs, patient medications and therapies. Provides education regarding disease, sign/symptoms, risk factor modification, medication management and diet.
  • Reviews stroke patient records for The Joint Commission Core Measures
  • Conducts post acute stroke discharge phone calls and follow-up on any coordination of care needs.
  • Maintains knowledge related to current clinical practices
  • Identifies stroke patients and refers to intake coordinator for Rehabilitation Services
  • Participates in bi-weekly rehabilitation conferences.
  • Coordinates and participates in coordination of community education and stroke support group.
  • Assists Stroke Program Coordinator and Medical Director with projects as required.
  • Performs responsibilities of Cath Lab RN when called back for neurology cases while on standby.
  • Responds to Code RRT and Code Blue, when not participating in active stroke or Cath Lab case.

 

Job Requirements

Required

  • 1-3 years Minimum 2 years acute care experienceas an RN in ICU or ER units
  • Current and Active Registered Nurse: CA
  • AHA Basic Life Support  Advanced Cardiac Life Support

 

Preferred

  • Critical care, cath lab, neurologyexperience
  • Experience in Patient CareCoordination
