Job Description

MAJOR ACCOUNTABILITIES/CRITICAL RESPONSIBILITIES:

  1. Provides professional nursing care, in collaboration with physicians for: assessment, implementation, follow-up, evaluation, and education for cardiac surgery patients
  2. Identifies patient/family individualized learning needs and implements measures to meet these needs based on patient/family capabilities to learn.
  3. Reports on activities and outcomes proactively.
  4. Maintains confidentiality and actively supports patient/family rights.
  5. Works collaboratively with colleagues to identify and resolve service issues and covers them as necessary.
  6. Coordinate with the multidisciplinary team including, floors, operating room, and ICU regarding cases
  7. Coordinate preoperative testing in a timely manner, aligned with POP clinic appointments and scheduled surgical dates
  8. Facilitate communication with other services (e.g., pulmonology) for surgical clearance.
  9. Complete and coordinate patient documentation (e.g., work letters, FMLA forms, clearance letters) for APP review and signature, ensuring follow-up with patients and families.
  10. Distribute weekly team communications outlining upcoming procedures, schedules, and meetings
  11. Conduct patient intake, including medication reconciliation and review of medical, surgical, and social histories
  12. Respond to patient phone inquiries related to routine Cardiac Surgery care, escalating concerns to the APP as needed and Coordinate scheduling and follow-up appointments.
  13. Monitor and respond to patient portal messages regarding routine questions or updates
  14. Ensure the collection and upload of outside medical records, imaging, and test results
  15. Accepts and completes other related duties as assigned.


QUALIFICATIONS/REQUIREMENTS:
Education:
Bachelors of Science in Nursing required

Experience: 3 years of experience as an RN in a Health Care setting and cardiovascular experience preferred

Possesses the skills to effectively communicate, problem-solve, and maintain productive and effective interpersonal relationships while effectively prioritizing.

Has the flexibility to adapt to new and changing situations.

Has the ability to think critically.

Able to give constructive feedback, supervises and teams with others, teaches staff and patients.

Possess basic computer skills.

License / Certification:
Current Connecticut RN license required.
BLS required
