This role serves patients throughout the Gilroy area and surrounding communities.
What You'll Do- Provide in-home speech therapy services for speech, language, cognitive-communication, and swallowing disorders
- Complete initial evaluations and develop individualized plans of care
- Deliver skilled therapy visits and reassess patient progress regularly
- Document patient progress and communicate changes to the care team and physician
- Collaborate with an interdisciplinary team to coordinate patient care
- Educate patients and caregivers to support independence and successful outcomes
- Complete required documentation, including OASIS when applicable
Who You Are- A licensed Speech-Language Pathologist who is compassionate, skilled, and patient-focused
- Comfortable working independently in a home health setting while collaborating with a supportive care team
- Dedicated to delivering high-quality, individualized speech therapy services that improve communication, swallowing safety, and quality of life
- Skilled at building meaningful relationships with patients and their families
- Organized, dependable, and confident in clinical decision-making and documentation
- Motivated by one-on-one care and helping patients achieve lasting, functional outcomes
Qualifications- Master's degree in Speech Language Pathology
- Active Speech Language Pathology license (CCCs)
- Ability to practice speech therapy unencumbered within state and federal guidelines
- Minimum of one year experience as a Speech-Language Pathologist (CFYs not accepted)
- Prior home health experience preferred
- Experience with Home Care Home Base (HCHB) documentation strongly preferred
