Qualifications

• MSW from an accredited school of Social Work required.
• Current membership with OASW and OCCSWSSW, or eligibility to obtain membership required.
• Experience in acute and/or sub-acute settings and discharge planning preferred.
• Strong computer skills, with ability to manage electronic documentation.
• Knowledge of community resources and patient flow within healthcare settings beneficial.
• Effective organizational skills to prioritize tasks in a fast-paced environment.
• Demonstrated ability to work both independently and as part of a team.
• Excellent interpersonal and communication skills for case reporting and assessments.

Responsibilities

• Assess psychosocial needs and provide crisis intervention to patients and families.
• Plan and coordinate patient discharges and facilitate patient flow across sectors.
• Liaise between patients and staff to connect with community resources.
• Collaborate with healthcare team members to ensure comprehensive care delivery.
• Manage a flexible caseload, adapting to varying program assignments.
• Supervise social work students during their clinical practice.
• Provide cross-coverage for Social Work Services during staffing relief.
