Full Job Description
Full time Social Work position on an inpatient Cardiac Medicine floor specializing in Heart Failure. Opportunity to utilize brief counseling skills working with patients facing acute and chronic illness, to support adjustment to illness and patient engagement in their treatment. Practice trauma informed care, motivational interviewing and family work. Collaborate closely with physician, nursing, rehab and case management colleagues as well as community-based agencies to help facilitate transition to appropriate level of care and optimize treatment outcomes.

Clinical supervision , including bi-weekly ICU Social Worker group and professional development are provided and highly valued. Continuing education and Social Work Grand Rounds provided on site.

Job Description:

Essential Responsibilities:

Provides clinical services to patients and families that address personal and environmental issues in order to maximize emotional, social, and physical well-being and the appropriate and efficient use of health care resources. Screens and triages patients who request service, are referred, or meet screening criteria for identifying patients' needs. Conducts psychosocial assessments and provides services as indicated. Partners with the multidisciplinary team to facilitate continuity of care planning. Meets productivity standards and expectations as set by the leadership in Social Work and the assigned clinical area. Provides counseling and psychotherapeutic services appropriate to setting. Provides timely and appropriate communication and documentation in the medical record. Provides consultation and education regarding psycho-social issues affecting patient care. Advises and addresses ethical and risk management issues. Presents and/or discusses clinical work in formal and informal settings, multi-disciplinary and clinical rounds and seminars. Participates in department or hospital process improvement and CQI efforts as needed. Partners with advanced staff on research. May presents clinical material and theory at teaching conferences and seminars Staff licensed as an LICSW may supervise staff or student interns and Serves as a content resource for multidisciplinary staff; and mentor/trainer to less experienced staff. Encourages an environment of idea sharing and feedback. Seeks opportunities and implements goals for own professional development.

Required Qualifications:

Master's degree in Social Work Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint, OneDrive and SharePoint.

Preferred Qualifications:

Relevant health care and/or human service experience.

Physical Nature of the Job:
Light work: Exerting up to 20 pounds of force frequently to move objects. Some elements of the job are sedentary, but the employee will be required to stand for periods of time or move throughout the hospital campus

 

Pay Range:

$69,971.20 USD - $113,006.40 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.
