Job DescriptionJob Title: Social Worker-Compressed Schedule (Overnights)- The social worker in this position will facilitate the discharge of patients in the ED.
Responsibilities- Responsibilities include psychosocial assessment; a full range of psychosocial interventions for patients/families; transitional care; and, when appropriate, follow-up phone calls; and post-discharge intervention.
- Services will include counseling; crisis intervention (including cases of domestic violence, child abuse and neglect, sexual assault, human trafficking, and death), assistance with concrete services (e.g., pharmacy, home care, transportation, entitlements, referral for community services), placement of patients in facilities and, close collaboration with social workers in outpatient settings and Care Coordination programs to whom patients may be referred for follow-up.
The ED social work team also provides coverage for urgent needs that come up after hours across the hospital.
This full-time position requires THREE 12.5-hour days weekly for three weeks/month and FOUR 12.5 hour days one week/month. The schedule will include weekend days on a rotating schedule. The hours for this position are overnight, 7 PM - 7:30 AM. Schedules are subject to change based on Emergency Department and inpatient flow. There is a salary supplement for working nights. During the orientation and training period, daytime hours will be required. For clinical supervision, there may be a need for adjustment of hours.
Qualifications- NYS LMSW or LCSW.
- Strong interest in being part of innovative initiatives to improve the quality of care for patients who present in ED/RETU.
- Excellent clinical skills, with exceptional skills in rapid psychosocial assessment, crisis intervention, and focused short-term treatment.
- Ability to prioritize effectively multiple, simultaneous requests for service and respond effectively to the urgent needs of a diverse patient population.
- Strong interdisciplinary/collaborative skills and organizational skills.
- Exceptional skills in engaging patients and families and expertise in coordinating resources within the hospital and community networks for effective management of patient care.
- Spanish speaking preferred.
- Internal candidates: Minimum one year in current position unless in the ED/ RETU.
Compensation StatementThe Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $88,691.00 - $101,670.78 Annually. Actual salaries depend on a variety of factors, including experience, education, and operational need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
