Responsibilities
The Senior VP - Population Health is accountable for translating the enterprise population health strategy into measurable improvements in clinical documentation, total cost of care, quality, and patient outcomes. This leader establishes the roadmap, leads a focused portfolio of high-impact clinical programs, and partners with Market Operations, Central Operations, and Medical Leadership to deliver performance across key population health measures. This role identifies the highest-value opportunities, builds scalable interventions, establishes clear accountability, and adjusts priorities and execution when performance is off track. The Senior Vice President builds and leads the population health function to deliver sustained results across the enterprise’s value-based care objectives.
Essential Duties and Responsibilities
- Define and maintain the enterprise population health strategy, roadmap, and priorities in alignment with the Executive Leadership Team.
- Build and lead a high-performing population health function with the capabilities, structure, and talent needed to execute the strategy.
- Lead the enterprise strategy for accurate diagnosis, documentation, and management of chronic conditions, emphasizing clinical integrity, specificity, completeness, and compliant coding.
- Partner with Medical Leadership, Market Operations, and Central Operations to improve Stars, HEDIS, preventive care, chronic disease outcomes, and patient experience.
- Own the clinical program portfolio in partnership with Medical Leadership, including prioritization, design, governance, and performance management, to improve total cost of care.
- Identify actionable cost drivers and develop interventions for high-risk populations, avoidable acute utilization, care transitions, medication-related risk, chronic disease management, and other significant opportunities.
- Establish clear performance measures and accountability for population health initiatives, monitor results, and lead corrective action when performance is off track.
- Oversee strategic vendors and external partners to ensure measurable value, effective integration, data quality, service performance, and scalability.
- Represent the organization with health plans, ACO partners, vendors, and other stakeholders on population health strategy and performance.
- Partner on continuing medical education (CME) session planning to support population health priorities.
- Participate in Quarterly Operating Reviews and Board of Directors meeting preparation and presentations, as appropriate.
- Travel intermittently to support business needs.
- Employees will be required to perform other job-related assignments as requested.
Qualifications
Required Knowledge, Skills and Experience
- Bachelor’s Degree
- 5+ years of experience in a senior healthcare leadership position
- Experience with HCC coding and documentation
- Experience with E/M coding and documentation
- Experience with prospective chart review programming
- Experience with clinical education
- Deep knowledge of quality program development and oversight
