Full-Time, leadership position to manage care
throughout our Northeast Ohio region, serving approx. 27 counties, with a central focus in Summit, Stark and Tuscarawas counties.
HIGHLIGHTS & REQUIREMENTS:This role requires a mix of Nurse Supervisory services and recertification visits - and travel - as needed to support all clients and staff across Northeast Ohio.- This role works closely with and reports directly to our NE OH RN Program Administrator (local) and Director of Clinical & Administrative Operations (statewide).
- For those pursuing career growth: a defined path for advancement will be supported into roles including: RNCM, Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions.
Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
- At least 1 year of experience is required in: Nursing Supervision; wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
- At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.

- Peer/staff education and training
- Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NE Ohio and our current + prospective employees in these communities.
SCHEDULE TYPE & SERVICE AREA: This is a
Full-Time, salaried position, requiring weekday, daytime availability (Monday - Friday, 8am - 5pm) and occasional evening or weekend work.
- As this is a "Road Warrior" Nursing Leadership position with our agency, travel is required as needed to all clients throughout Northeast Ohio (approx. 2-hour radius service area) across approx. 27 counties. Willingness to travel across this region is required.
- Typical travel distances include a 60-minute radius daily + up to 2 hours on occasion.
- Door-to-Door mileage reimbursement provided.

COMPENSATION:- Full-time Annual Salary* (+ PTO + Mileage) starts between $80,000 - $85,000 per year .

Job SummaryAs an RN Supervisor, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.
- With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
- You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
- You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
Responsibilities/Requirements of a Clinical Supervisor:- RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
- RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
- RN to Document/Communicate/Coordinate patient status/change in condition to care team.
- RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
- Willing to travel surrounding counties.
- Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.
Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing
Qualifications- Graduate of an accredited School of Nursing.
- Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
- CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
- At least 1 year of experience is required in: Nursing Supervision; wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
