Description:
Department: Case Management
Required Certifications: BLS
Other Certifications/Specialty certifications requirements: PRI & SCREEN certifications (can be taken online).
Role: Expected to have knowledge of discharge planning and making referrals.
Will typically be working on a medical-surgical or telemetry unit and will share responsibilities with a Social Worker.
Must be able to function independently on the unit while collaborating with an interdisciplinary team and patients for discharge planning.
Expected to round on the unit with physicians and the healthcare team.
Utilization Review is a separate department; they handle insurance approvals and arrange transportation.
Caseload on floors typically ranges from 17 to 25 patients, with some pilot floors having a caseload of 12 to 17 patients.
Need to be able to manage at least 20 patients.
Experience with EMR: Allscripts is preferred.
Knowledge of CarePort, MCG, and/or InterQual is preferred.
This position focuses primarily on discharge planning for inpatient care.
Common types of patients include those being discharged to home, community, or skilled nursing facilities.
Responsibilities also include obtaining housing resources for patients, shelter packets for homelessness, and referrals for specialized wound care.
Experience with prior critical care is preferred.
Management is supportive of taking a full lunch break during shifts.
Shifts are Monday through Friday.
Occasional weekend and holiday coverage may be required with prior approval from management.
Dress code is business casual or any color scrubs, with certain colors discouraged.
Department-specific orientation lasts for 2 days, followed by training with a preceptor.
