Job Description:- Compiles data, tracks outcomes, and makes recommendations for process improvement.

- Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services.
- Ensures seamless transitions across the continuum of care.
- Educates patients and families about post-acute care services and expectations.
