RN Case Manager Nurse - HospiceAmedisys Hospice of Brevard
**This position is based in Melbourne, FL and requires daily travel within
South Brevard County. Schedule:
Monday-Friday, 8:00 AM - 5:00 PM. Salary Range: $83,000-87,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & TrainingYou will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.
Responsibilities
- Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
- Records pain/symptom management changes/outcomes as appropriate.
- Teaches patients, families, and caregivers appropriate patient care techniques.
- Regularly communicates with clinical manager and care team.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Other duties as assigned.
Qualifications
- Current RN license, specific to the state(s) you are assigned to work.
- 1+ years of clinical RN experience in acute healthcare setting(s)
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
- Candidates must currently reside in Rockledge or north of Brevard County.
Preferred:- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
