Responsibilities

  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and services to patients/families as assigned.
  • Other duties as assigned.

RN Case Manager Nurse - Hospice

Amedisys Hospice of Brevard

**This position is based in Melbourne, FL and requires daily travel within South Brevard County. Schedule: Monday-Friday, 8:00 AM - 5:00 PM.

Salary Range:

$83,000-87,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.

  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.


Qualifications

  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
  • Candidates must currently reside in Rockledge or north of Brevard County.

  • Strong communication and interpersonal skills
  • Ability to work independently and manage multiple priorities
  • Comfortable with daily travel between facilities
  • Proficient in electronic medical record (EMR) systems
