$500 CONTRACT COMPLETION BONUSOpportunity: RN - Home Health Nurse
Job Details:• 13-week contract with 40 guaranteed hours per week
• 8-hour day shifts, roughly 8:00 AM to 4:30 PM with flexibility in start and end times depending on census
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Travel required covering 20-25 patients with an average of 4.5 visits per day, ranging from 25-60 miles per day depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Potential to float to Medford, Antigo, and WI Rapids on a limited basis
• Responsibilities include OASIS admissions, all time points, episode management, physician calls, coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Required certifications: AHA or ARC BLS, active WI or Compact license, valid driver's license; OASIS certification a plus
• Minimum 2+ years of home health experience required
• Prior travel experience required
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Required education: Associate Degree or Certificate; Bachelor's Degree preferred
• Scrub color: Navy blue
• Traveler must reside more than 50 miles from facility
