Job Details:• Provide home health physical therapy services across York and Lancaster counties in South Carolina
• Approximately 6 total patient visit points per day
• May occasionally be asked to float into Union or Mecklenburg County
• Weekend-only position may require driving up to 100 miles depending on caseload
• Treat adult patients only with chronic diseases, wound care, IV, pleurx, and wound vac needs
• Perform OASIS-E assessments and provide disease teaching and chronic disease management
• Complete maintenance notes and assessment documentation
• Charting experience with Homecare Homebase EMR is required (no exceptions)
• Experience with OASIS-E assessment tool required
• Skills required include IV, pleurx, wound vac, wound care, assessment, and disease teaching
• Experience with geriatric and adult populations and chronic disease management required
• Equipment used includes Samsung Note 9 phones and Homecare Homebase EMR system
• Scrub color: Grey - Therapy
• 5-day orientation (NEO) required
• Minimum 1 year of experience required; first-time travelers accepted if seasoned in home health
• Required certifications: BLS
• South Carolina state licensure required
• Minimum education requirement: Bachelor's Degree
• Valid driver's license and proof of insurance required
• Traveler's permanent address must be 50 miles outside of the home unit
