Job Description

The RN Clinical Navigator will be responsible for providing clinical care and guidance to high-risk patients in order to promote better health outcomes. The position will work to eliminate barriers to care by guiding, educating and navigating patients to receive necessary health care monitoring and resources, both within Centra and externally. The position will be charged with building professional relationships with the patient and their families, local and regional healthcare providers and vendors to ensure smooth transitions and timely access to care for patients. The practice of the professional nurse reflects the American Nurses Association's Code of Ethics for Nurses and the Nurse Practice Act of Virginia.

Responsibilities

Provides guidance and education appropriate to the patient demographic and clinical needs;

Collaborates and shares information with identified community outreach programs and other identified resources.

Connects the patients to programs that are specific for the continuation of their care and empowers patients to take charge of their health;

Manages programs and resources within the clinic or healthcare setting, and applies disease management science in all aspects of care delivery;

Ensures timeliness in scheduling appropriate physician, specialty provider, diagnostic and/or support services;

Manages a repository of patient information that provides an overview of the patient demographic and health information to be used by multidisciplinary response teams and other community resources.

Makes appropriate referrals to community resources and serves as a liaison to the health care team;

Assists patients and families to gaining access to culturally appropriate, supportive care; Promotes informed decision making among patients and family members;

Assists medical/care team in identifying patients' problems/needs and lead efforts to set priorities, identify problems requiring follow-up or further medical referrals;

Utilizes the nursing process, as a frame of reference for the delegation and coordination on patient education needs within the scope of nursing practice; Engages in community collaboration, education and outreach;

May perform other duties as assigned or requested and job specification can be modified or updated at any time.

Qualifications

Required Education: Graduate of an accredited Nursing Program

Preferred Education: Bachelors of Science Degree in Nursing (BSN)

Required Experience: At least three (3) years of nursing experience. Maintain annual competencies and licensure requirements.

Required Certifications and Licensures: Hold a current, active license as a registered nurse in Virginia or hold a current multistate licensure privilege as a registered nurse. Hold a current, active American Heart Association Basic Life Support (AHA BLS) course completion card. PACE RN Clinical Navigator requires LTSS (Long term services and supports screening) certification to be obtained within the first 30 days of hire and to be maintained throughout employment.

Preferred Certifications and Licensures: Specialty practice or certification in appropriate field

Salary Range: $74,393.00-$111,590.00/Annually
