Qualifications

• Associate or bachelor's degree in nursing.
• Minimum one year clinical RN experience in home health or hospice.
• Minimum one year RN management experience.
• Current RN license for the assigned state(s).
• Current CPR certification.

Responsibilities

• Support and collaborate with the staffing director.
• Ensure high-quality patient care and clinical performance of the nursing staff.
• Organize clinical operations at the care center.
• Review service requests and determine patient eligibility for home care.
• Oversee scheduling for staff and patient visits.
• Manage payer verification and precertification processes.
• Address and resolve documentation issues from team members.
• Assist clinicians in creating and updating patient care goals and plans.
• Evaluate patient needs and adjust care plans accordingly.
• Provide direct patient care occasionally during emergencies.
• Engage in the on-call process as needed.
• Train staff to uphold quality care and compliance standards.
• Handle patient and family concerns effectively.
