Clinical Manager (RN) - Home HealthFocus Home Health - Stockton, CAWe are seeking an experienced full-time Clinical Manager (RN) to lead our Stockton clinical team. This leadership role is responsible for overseeing the delivery of high-quality patient care, supporting and developing clinical staff, ensuring regulatory compliance, and partnering with interdisciplinary teams to achieve exceptional patient outcomes.

What You'll Do- Provide leadership, supervision, and support to a team of home health clinicians.
- Coordinate the delivery of high-quality patient care while promoting positive patient outcomes.
- Ensure compliance with all federal, state, local, Medicare, and Medi-Cal regulations.
- Conduct and facilitate interdisciplinary care conferences, team meetings, and clinical in-service education.
- Review referrals and coordinate timely patient assessments and admissions.
- Make appropriate patient care assignments while overseeing clinician scheduling and on-call coverage.
- Monitor clinical documentation and quality standards to ensure regulatory compliance.
- Maintain accurate records related to patient assignments and clinical management.
- Serve as a clinical resource, mentor, and preceptor for nursing staff when needed.
- Foster a collaborative, supportive culture focused on excellence in patient care.
What You'll Bring- Current California Registered Nurse (RN) license in good standing.
- Graduate of an accredited school of nursing.
- Minimum of two (2) years of clinical nursing experience within the past five (5) years.
- Previous home health leadership or supervisory experience preferred.
- Strong knowledge of Medicare Conditions of Participation and home health regulations.
- Excellent communication, organizational, and leadership skills.
- Ability to build positive relationships with patients, families, referral partners, and team members.
- Valid driver's license, reliable transportation, and current automobile insurance.
