Full Job Description
  • Full-Time | Day Shift Monday-Friday
    Salary: $83,000-90,000 per year
  • We are seeking a dedicated Registered Nurse (RN) Case Manager who is passionate about hospice care and committed to delivering exceptional clinical and emotional support to patients and families.
    RESPONSIBILITIES
    The RN Case Manager will:
    • Perform initial and ongoing comprehensive nursing assessments for hospice patients.
    • Provide compassionate clinical care, including symptom management, treatments, and preventive nursing procedures.
    • Deliver dietary counseling and patient/family education to support comfort and quality of life.
    • Observe and promptly report significant changes in patient condition to the physician and interdisciplinary team.
    • Collaborate with the IDG to develop, update, and implement individualized care plans.
    • Teach, supervise, and participate in the ongoing training of nursing personnel.
    • Maintain clinical documentation in compliance with Harmony Hospice standards, Medicare and HIPAA regulations.
    • Participate in on-call rotation to ensure 24/7 patient support.
    • Meet agency infection control, quality, and compliance standards.
    • Attend required meetings, team conferences, and continuing education.
    • Perform additional duties as assigned by the Director.

    SCHEDULE
    • Five 8-hour days (Monday-Friday)

    This is a road-based position serving multiple counties within our service area on the West side of Cleveland and surrounding areas
