What You'll Do- Provide skilled nursing support across multiple home health territories throughout the Twin Cities.
- Complete OASIS assessments and individualized plans of care.
- Deliver one-on-one skilled nursing visits and patient education.
- Collaborate closely with physicians and interdisciplinary team members.
- Support local teams during staffing needs, growth, and coverage.
- Maintain accurate and timely clinical documentation.
What We're Looking For- Active Minnesota Registered Nurse (RN) license.
- Strong clinical assessment and critical thinking skills.
- Comfortable adapting to different territories and patient populations.
- Home health experience preferred, but not required.
- Reliable transportation and willingness to travel throughout the metro.
