Role SummaryThe RN Care Manager (RN-CM) Preceptor serves as a clinical educator, mentor, and subject matter expert for the RN Care Manager program. This role is responsible for onboarding and training newly hired RN-CMs, validating clinical competencies, supporting workflow adoption, and promoting best practices across the care management team. The RN-CM Preceptor partners closely with operational leadership to ensure RN-CMs are equipped with the knowledge, skills, and confidence necessary to deliver high-quality care management services that improve patient outcomes and support organizational goals.Key ResponsibilitiesOnboarding and Training- Lead onboarding and orientation for newly hired RN Care Managers.
- Develop and maintain standardized onboarding curricula, training materials, job aids, and learning resources.
- Facilitate virtual and in-person training sessions on RN-CM workflows, care management principles, documentation standards, and technology platforms.
- Coordinate and oversee shadowing experiences and structured learning plans.
- Monitor onboarding progress and provide regular feedback to new hires and leaders.
- Validate readiness for independent practice through competency assessments and skills demonstrations.
Competency Management- Develop and maintain RN-CM core competency standards.
- Conduct annual competency assessments and skills validations.
- Identify knowledge gaps and create targeted development plans.
- Support remediation plans for RN-CMs requiring additional coaching or performance support.
- Ensure consistency in practice standards across markets and regions.
Clinical Practice Leadership- Serve as a subject matter expert for RN-CM workflows, care coordination, chronic disease management, care planning, transitions of care, and patient engagement.
- Promote evidence-based care management practices.
- Support adoption of new workflows, programs, and technology enhancements.
- Participate in pilot programs and assist with change management efforts.
- Provide consultation and guidance on complex patient cases when needed.
Coaching and Mentorship- Provide ongoing mentorship and professional development support for RN-CMs.
- Foster a culture of continuous learning and clinical excellence.
- Facilitate peer learning opportunities, case reviews, and best-practice sharing.
- Support career development and succession planning efforts within the RN-CM program.
Quality and Performance Improvement- Monitor onboarding and competency outcomes to identify improvement opportunities.
- Analyze performance trends and recommend educational interventions.
- Partner with operational leaders to improve quality metrics, care management outcomes, and documentation accuracy.
- Support audit and compliance activities related to RN-CM practice standards.
Collaboration- Partner with RN-CM leaders, Clinical Operations, Population Health, Learning & Development, and other stakeholders to ensure alignment of training and competency programs.
- Collaborate with interdisciplinary teams to support integrated care delivery.
- Participate in organizational committees and workgroups as assigned.
QualificationsRequired- Active Registered Nurse (RN) license in good standing.
- Bachelor's degree in Nursing (BSN).
- Minimum 3 years of clinical nursing experience.
- Minimum 2 years of care management, case management, population health, or chronic disease management experience.
- Demonstrated excellence as an RN Care Manager.
- Strong presentation, facilitation, coaching, and communication skills.
- Experience training, mentoring, or precepting staff.
- Proficiency with electronic medical records and care management platforms.
Preferred- Certified Case Manager (CCM) or other relevant certification.
- Experience developing training curricula and competency programs.
- Experience supporting large-scale operational initiatives or system implementations.
- Previous leadership, educator, or preceptor experience.

Pay RangeThe typical pay range for this role is:
$66,575.00 - $142,576.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
