Job Summary and Responsibilities

As our RN Care Manager, you will coordinate and facilitate comprehensive care plans for a diverse patient population, ensuring seamless transitions and optimal health outcomes.Every day you will assess patient needs, develop individualized care plans, and collaborate with patients, families, and multidisciplinary healthcare teams to integrate medical, social, and psychological aspects of care. You will serve as a central point of contact, advocating for patients, facilitating access to resources, and proactively managing their continuum of care to prevent complications and reduce hospital readmissions.To be successful in this role, you will demonstrate exceptional clinical knowledge, superior communication and advocacy skills, and a strategic, patient-centered approach to healthcare delivery that empowers individuals to achieve their highest level of wellness.

Job Requirements

Required

  • Current Washington State licensure as an RN
  • CPR certification is required every two (2) years
  • Bachelors of Science of Nursing
  • Strong patient assessment skills and the ability to work collaboratively in team setting
  • Current working knowledge of electronic medical records
