Registered Nurse Admissions
We are seeking a compassionate and skilled Registered Nurse Admissions Nurse to join our dedicated team of caregivers at Van Duyn Center for Rehabilitation and Nursing in Syracuse, New York. As a key member of our admissions team, you will play a vital role in providing exceptional care and service to our residents and their families.
Responsibilities:
• Conduct comprehensive nursing assessments to identify the needs of potential residents and develop individualized care plans
• Collaborate with interdisciplinary teams to develop and implement care plans that address the physical, social, emotional, and spiritual needs of residents
• Provide education and support to residents and their families on various aspects of care and troubleshooting
• Ensure adherence to regulatory standards and company protocols to maintain optimal quality of care
• Develop and maintain positive relationships with residents, families, and healthcare professionals to ensure a seamless transition into our care community
• Participate in quality improvement initiatives to continuously enhance the delivery of care and services
