Position Overview

Central Maine Healthcare is seeking a Full-Time Rheumatology Advanced Practice Provider. The Rheumatology Advanced Practice Provider (APP) provides comprehensive care to patients with autoimmune, inflammatory, and musculoskeletal disorders in collaboration with supervising rheumatologists. The APP is responsible for patient evaluation, diagnosis, treatment planning, medication management, patient education, and ongoing monitoring of chronic rheumatic diseases. This role supports high-quality, patient-centered care across outpatient and, when applicable, inpatient settings.

Responsibilities Clinical Care
  • Perform comprehensive patient assessments, including medical histories, physical examinations, and diagnostic evaluations.
  • Evaluate and manage patients with rheumatologic conditions such as:
    • Rheumatoid Arthritis
    • Psoriatic Arthritis
    • Ankylosing Spondylitis
    • Systemic Lupus Erythematosus
    • Vasculitis
    • Gout
    • Osteoporosis
  • Order, interpret, and communicate laboratory tests, imaging studies, and other diagnostic procedures.
  • Develop and implement individualized treatment plans under established clinical protocols.
  • Prescribe medications and therapies in accordance with state regulations and organizational policies.
  • Monitor treatment efficacy, adverse effects, and disease progression.
Medication and Infusion Management
  • Initiate and manage conventional, biologic, and targeted therapies.
  • Monitor patients receiving immunosuppressive medications and biologic agents.
  • Collaborate with infusion centers, specialty pharmacies, and insurance providers regarding medication authorizations and treatment plans.
  • Ensure appropriate laboratory monitoring and medication safety practices.
Patient Education
  • Educate patients and families regarding disease processes, treatment options, medication administration, and lifestyle modifications.
  • Promote adherence to treatment plans and preventive health measures.
  • Provide counseling regarding symptom management and self-care strategies.
Care Coordination
  • Collaborate with physicians, nurses, pharmacists, physical therapists, and other healthcare professionals.
  • Coordinate referrals and consultations with specialists as needed.
  • Participate in multidisciplinary case reviews and quality improvement initiatives.
  • Assist with transitions of care and follow-up management.
Documentation and Compliance
  • Maintain accurate, timely, and complete electronic medical records.
  • Ensure compliance with organizational policies, clinical guidelines, and regulatory requirements.
  • Participate in quality assurance, patient safety, and performance improvement activities.
Qualifications
