Full Job Description
We are currently seeking a full-time Registered Nurse to join our outpatient CCBHC team in Hartsdale. The RN plays a critical role in ensuring the coordination of services related to medical conditions, screening/monitoring patients for gaps in preventative services, and physical health conditions related to behavioral health conditions and their co-morbidities. The RN provides screening and monitoring of key health indicators and health risks, as well as clinical monitoring for adverse effects of medications, including monitoring for metabolic syndrome and movement abnormalities. The RN will consult with other team members and coordinate care between the psychiatric staff and clinical team. The RN will utilize evidence-based practices and practice within a trauma-informed and person-centered approach. The salary range for this full-time position is $80,000 - $90,000. Responsibilities: As a valued member of our team, your responsibilities will include, but are not limited to: • Oversee coordination of care for physical health and wellness, including providing referrals to community providers, scheduling appointments, and liaising with medical providers to obtain records. • Provide and supervise psychiatric nursing support throughout the agency including but not limited to Article 31 and 32 CCBHC clinics, OnTrackNY, and supporting psychiatric services for group home residents. • Maintain medical equipment up to date, and follow a regular preventive maintenance schedule which includes: cleaning, calibrating, inspecting, and replacing worn parts as needed. • Completion of prior authorizations as needed for specific medications and assist with completion as needed. • Ensure that electronic health records are up to date with lab results and medical reports from outside facilities. • Manage triage system/issues as they arise including managing medication refill requests. • Arranging for physical examinations of clients who have not had one within one year prior to program admission, scheduling appointments, and liaising with medical providers to obtain records. • Acts as Health Coordinator for the program, providing health education, risk reduction, counseling, and referral service to all clients, specifically HIV/AIDS, tuberculosis, viral hepatitis, sexually transmitted diseases, and other communicable diseases. • Coordinate, administer and monitor usage of prescribed MAT/MOUD therapies. • Conduct toxicology testing, as needed. • Collaborate with the prescriber on medication refills. • Coordinate and monitor usage of prescribed psychiatric medication such as clozapine (obtaining lab results, reviewing them, and faxing prescriptions) and administration of medications via intramuscular and subcutaneous injections as needed. according to Rx order(s) as documented in EHR. • Follow the established agency policies and maintain appropriate professional standards. • Ensure client engagement in health and wellness practices, as well as chronic disease management, to reduce hospitalizations and improve quality of life. • Coordinate care with outside pharmacies and providers. Requirements: We are seeking candidates who possess the following qualifications: • Bachelors degree in Nursing and be actively licensed in New York State in good standing. • Computer knowledge and ability to document in an electronic health record is required. • Cleared by NYS Justice Center through fingerprinting for a Criminal History Records search. • Cleared by the Justice Center for the Protection of People with Special Needs Staff Exclusion List (SEL) and the New York State. • Ability to work collaboratively with a multidisciplinary team. • Experience in behavioral health. • Deep understanding family systems based on diversity of cultural and ethnic backgrounds. • Experience with and understanding of various evidence-based models and theories pertaining to psycho-education, motivational interviewing, serious mental illness and recovery. • Extensive knowledge of mental health, substance use, family support services, and community resources. • Use of Motivational Interviewing, recovery and trauma-informed approaches when delivering services. • Reliable, efficient, and able to work independently and understands the importance of maintaining confidentiality.
