Facilitate discharge planning and arrange post-acute discharge services
Conduct follow-up after patients return to their discharge destinations

Nurse Unit Manager
We are seeking a Registered Nurse - RN - Unit Manager.
$90K to $95K annually based oncircumstances including skills, qualifications, degrees and certifications, prior job experience, training, market data, and other relevant factors.
Registered Nurse - RN Unit Manager Qualifications:
Valid Illinois Nursing License as an RN - Registered Nurse.
Minimum of 2 years of nursing supervisory experience.
Minimum of 2 years of SNF and LTC experience.
Have current CPR certification.
