Registered Nurse (RN) - PM & NOC Shifts

Sunnyvale Gardens Post Acute | Sunnyvale, CA

Sunnyvale Gardens Post Acute is seeking dedicated and compassionate Registered Nurses (RNs) to join our clinical team for PM and NOC shifts. Our 140-bed skilled nursing and rehabilitation facility is committed to providing high-quality, resident-centered care in a supportive and collaborative environment.

Position Highlights
  • Job Type: Full-Time
  • Pay Rate:$50.00-$58.00 per hour, DOE
  • Shifts: PM & NOC
  • Schedule: 4 days on, 2 days off - rotating schedule
  • Benefits: Comprehensive benefits package, including Medical, Dental, Vision, Paid Time Off, and Retirement/401(k) options

Key Responsibilities
  • Provide compassionate, high-quality clinical care to short-term rehabilitation and long-term care residents.
  • Administer medications, treatments, and IV therapies safely and accurately.
  • Perform resident assessments and monitor for changes in condition.
  • Accurately document clinical findings, interventions, and resident status.
  • Provide direction and support to CNAs and LVNs.
  • Communicate effectively with physicians, residents, families, and the interdisciplinary team.
  • Respond appropriately to clinical concerns and emergencies.
  • Promote a safe, respectful, and resident-centered environment.

Qualifications
  • Current, unrestricted California RN license.
  • Current BLS/CPR certification.
  • Previous skilled nursing, post-acute, rehabilitation, or long-term care experience preferred.
