complexity of acute inpatient nursing, supported by a collaborative interdisciplinary team.

- Local Travel Only (within estimated ~30 minutes of Lutheran General Hospital)
- Manageable Caseload: 3-4 patient assignments, which may include admits/discharges, per 12-hour shift

What You'll Do- Deliver acute, hospital-level care in patients' homes
- Perform skilled clinical tasks (IV starts, phlebotomy, medication administration)
- Monitor patient conditions and recognize changes requiring escalation
- Collaborate with a virtual and in-person interdisciplinary team
- Use evidence-based practice to ensure safe, patient-centered care
- Coordinate discharge planning and ongoing care needs
- Promote a culture of safety and high-quality outcomes
What We're Looking For:- Active RN license in the state of Illinois
- BLS certification (AHA)
- Valid driver's license with a clean driving record
- 2+ years of recent acute inpatient care or ED experience preferred
- Strong IV, phlebotomy, and clinical assessment skills
- Ability to work independently in home settings
- Excellent communication and critical thinking
- Comfortable with technology and remote monitoring tools
Work Environment- Travel within designated service area
- Provide care in private home settings
- Physical activity required (lifting, bending, standing, etc.)
- Exposure to clinical and environmental variables typical in-home care
