Responsibilities

  • Manage and document patient records, including OASIS and progress notes

Full Job Description

What You'll Do
  • Perform comprehensive assessments in the patients' homes and identifies patient's needs
  • Develop a care plan in collaboration with patient/CGs, healthcare provider, case manager, and/or supervisor as assigned and document appropriately
  • Assess the need for additional services and equipment on admission on an ongoing basis and inform the case manager and/or supervisor on a timely basis
  • Assess the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects
  • Take verbal orders from the healthcare provider as necessary promptly and documents appropriately
  • Initiate appropriate preventative, rehabilitative, therapeutic, comfort, and pain interventions and provide instructions to patients/CGs and staff promptly
  • Demonstrate professionalism by maintaining high standards of clinical care and staying current on clinical and medication issues
  • Updates to patients' clinical records including medication profiles, plan of care, OASIS, case conference, progress notes, and re-certification documentation as required
  • Serve as a preceptor when needed
