Your Clinical ImpactOversee Individualized Healthcare- Complete nursing assessments and monitor the health status of individuals receiving residential services.
- Create, carry out, and evaluate individualized healthcare plans designed to promote wellness and quality of life.
- Coordinate healthcare services with physicians, specialists, therapists, pharmacies, and other medical professionals.
- Advocate for individuals during medical appointments and ensure recommended follow-up and continuity of care.
- Identify changes in health status, address emerging concerns, and provide appropriate nursing interventions.
Strengthen the Healthcare Team- Supervise and provide support to Licensed Practical Nurses (LPNs), Medical Case Managers, Certified Nursing Assistants (CNAs), and Certified Home Health Aides (CHHAs).
- Provide residential staff with medication administration training, clinical education, and ongoing nursing guidance.
- Act as a clinical resource for interdisciplinary teams by providing consultation and recommendations related to individual healthcare needs.
- Promote collaboration, continued learning, and person-centered healthcare practices among clinical and support staff.
Advance Clinical Quality- Review medical records and documentation to promote accuracy, completeness, and compliance with nursing standards and agency policies.
- Participate in quality improvement efforts focused on strengthening healthcare services throughout Residential Programs.
- Help evaluate clinical procedures, nursing protocols, and regulatory requirements to support consistent adherence to best practices.
- Provide clinical consultation to vocational and day program services when healthcare needs arise.
Coordinate Care & Build Partnerships- Work collaboratively with families, guardians, physicians, hospitals, behavioral health providers, support coordinators, and community partners.
- Keep agency leadership and interdisciplinary teams informed about medical concerns, treatment plans, and ongoing healthcare needs.
- Participate in audits, licensing reviews, investigations, and regulatory inspections.
- Support compliance with DDD, DHS, DCF, and other applicable standards and requirements.

RequirementsRequired Qualifications- Graduate of an accredited School of Nursing.
- Current Registered Nurse (RN) license issued by the State of New Jersey.
- Valid New Jersey driver's license in good standing with no more than five (5) motor vehicle points.
- At least one (1) year of recent medical-surgical nursing experience.
- Strong clinical assessment, communication, organization, leadership, and decision-making skills.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Completion of all required DDD training requirements and maintenance of current CPR and First Aid certifications.
- Successful completion of all required background screenings, including CHRC, CARI, Central Registry, and applicable drug testing requirements.
- Ability to meet the physical demands of the position, including lifting, transferring, or assisting with up to 50 pounds when necessary.
- Prior experience supporting adults with intellectual or developmental disabilities preferred.
