Job DescriptionThe RN Care Manager is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. You'll help to provide continuity of care for positive outcomes in our family centered environment.
Key Responsibilities:- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team.
- Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs and the resources available.
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the SHC system to jointly communicate, problem solve, and share accountability for optimal outcomes.
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care.
- Ensures prudent utilization of all resources by evaluation of the resources available to the patient and balancing cost and quality to ensure optimal clinical and financial outcomes.
- Is accountable for the achievement of optimal outcomes within the standards of practice. Contributes to decision-making and decision support as a member of the interdisciplinary team by ensuring timely sequencing of care and services throughout patient encounters.
- Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy.
Required Qualifications:- BSN
- RN license in state of practice
- 3 years Nursing experience
Preferred Qualifications:- MSN
- Case Management Certification (CMC, ACM or ANCC) is required, or willingness to achieve within 12 months of eligibility.
- 5+ years of clinical experience in Pediatrics, Burn care and/or Orthopedics
- 1 year Care Coordination/Case Management
- Experience with utilization review/management with a working knowledge of InterQual criteria
The pay range for this position is $84,676.80 to $127,004.80.
Compensation is determined based on years of relevant experience and departmental equity.
