Full-Time - Day Shift

The Palliative and Hospice Nurse Navigator provides very specific expertise to individuals, family's and care givers. In this role, primary job responsibilities to include patient Advocacy and support, care coordination, education and communication, program development and quality improvement, psychosocial development and end of life care and support. This role is integral to providing patient centered and compassionate care to individuals, family's and care givers and making a profound impact in their lives.

Work Experience

Key Responsibilities
  • Assess Patient, Family and Care Giver needs
  • Provide compassionate and sensitive support and communication with patients, families, and health care teams regarding terminal illness and disease processes
  • In-depth understanding of palliative care principles, hospice philosophy, hospice eligibility and end of life care
  • Conduct comprehensive patient assessments and develop treatment plans
  • Assist with symptom management and pain management strategies
  • Facilitate goals-of-care and advance care planning discussions
  • Collaborate with multidisciplinary teams to optimize patient outcomes
  • Support patients and families through complex medical decision-making
  • Maintain accurate and timely documentation within the electronic medical record
  • Knowledgeable on palliative and hospice quality measures

Qualifications
  • Bachelors or higher degree from an accredited Registered Nurse program
  • Registered Nurse licensed in the state of Maryland
  • Must have CHPN or obtain within 1 year of hire
  • Prior palliative care, hospice, oncology, ICU, or acute care experience preferred
  • Minimum 2 years registered nurse experience required; must have at least 2 years of hospice experience
  • Strong communication and interdisciplinary collaboration skills
