Work Experience Key Responsibilities- Assess Patient, Family and Care Giver needs
- Provide compassionate and sensitive support and communication with patients, families, and health care teams regarding terminal illness and disease processes
- In-depth understanding of palliative care principles, hospice philosophy, hospice eligibility and end of life care
- Conduct comprehensive patient assessments and develop treatment plans
- Assist with symptom management and pain management strategies
- Facilitate goals-of-care and advance care planning discussions
- Collaborate with multidisciplinary teams to optimize patient outcomes
- Support patients and families through complex medical decision-making
- Maintain accurate and timely documentation within the electronic medical record
- Knowledgeable on palliative and hospice quality measures
Qualifications- Bachelors or higher degree from an accredited Registered Nurse program
- Registered Nurse licensed in the state of Maryland
- Must have CHPN or obtain within 1 year of hire
- Prior palliative care, hospice, oncology, ICU, or acute care experience preferred
- Minimum 2 years registered nurse experience required; must have at least 2 years of hospice experience
- Strong communication and interdisciplinary collaboration skills
