Nurse Manager (RN) -Memory Care

Monday-Friday | 7:00 AM-4:00 PM | Limited On- Call Rotations

We are searching for a Nurse Manager who leads with empathy, prioritizes person-centered care, and is deeply committed to helping our members preserve their dignity, comfort, and quality of life.

About the Role

As a Nurse Manager, you'll serve as the clinical leader for your neighborhood, ensuring exceptional resident care while mentoring and developing nursing staff. You'll partner with interdisciplinary teams, residents, and families to create an environment where quality of life and quality of care go hand in hand.

What You'll Do

Lead Clinical Excellence:
  • Provide daily leadership and oversight for your assigned nursing neighborhood
  • Conduct rounds, audits, and quality reviews to ensure outstanding resident care
  • Monitor compliance with clinical standards, policies, and regulatory requirements
  • Champion continuous quality improvement initiatives

Develop and Support Your Team:
  • Coach, mentor, and develop nursing staff
  • Conduct performance evaluations and provide ongoing feedback
  • Facilitate staff education, training, and professional growth
  • Foster a positive, collaborative team environment through regular communication and engagement

Partner with Residents and Families:
  • Build strong relationships with residents and their loved ones
  • Communicate changes in care plans and resident status
  • Ensure every resident receives compassionate, individualized care that promotes dignity and quality of life

Collaborate Across Disciplines:
  • Work closely with physicians, rehabilitation staff, social work, and other healthcare professionals
  • Participate in care planning and interdisciplinary meetings
  • Support survey readiness, quality assurance initiatives, and regulatory compliance effort

What We're Looking For
  • Current New York State RN License
  • Minimum 5 years of progressive nursing experience
  • Minimum 3 years of Long-Term Care experience
  • Experience caring for residents with dementia and memory-related conditions
  • Strong leadership, communication, and organizational skills

Preferred
  • BSN
  • Prior nurse leadership or management experience
  • Experience with MDS and care planning
