About the role
This role will be based in 2 programs: Latina's y Nino's program (LYNC) aims to serve women in recovery. Latinas y Niños is a 6- to 12-month addiction treatment program. Latinas y Niños focuses on the unique cultural and social needs of Latina women in recovery and their children, and the Familias Unidas Outpatient Program (FUOP) is a licensed, CARF-accredited behavioral health clinic. Familias provides clinical treatment for substance use disorder(s) (SUDs) and co-occurring SUD(s) and mental illness(es) (CODs). Each individual or family we serve receives a comprehensive assessment and CasaCare treatment plan. Our assessments identify each patient's health needs and explore their strengths and goals. Each patient consults with a multidisciplinary team to develop their CasaCare plan.

As part of the Casa Esperanza, Inc. team, you will provide services in a fully integrated, holistic, and collaborative manner using a team-treatment approach to ensure the coordination of all client services, both across Casa programs and with external providers. This integrated model aims to facilitate clear communication amongst all providers and eliminate duplication of efforts or conflicting approaches to care; detail necessary follow-up and assignments; and ensure wrap-around support services are in place to maximize treatment effectiveness and clinical outcomes.

What you'll do

For Latinas y Niños Program

  • Complete Nursing Assessments, COWS, CIWA Assessments
  • Complete medical clearance exam at time of intake
  • Complete Medication Monitoring and all required documentation
  • Complete medication counts at the start and finish of the shift of all control substances.
  • Work closely with the clinical team during MDT meetings to offer and provide client medical insight as part of case presentations.
  • Consult with staff and attend case conferences, as needed
  • Work collaboratively with PMHNP/ Providers for medication reconciliation process.
  • Work as part of an integrated care team model
  • Directly works in collaboration with community providers for medication refills and reconciliation.
  • Ensures a clearly reviewed and record of all quality assurance reviews
  • Establishes group curriculum to provide 2 hours weekly of health education material
  • Meets with each patient to establish medication plans as needed for safety intervention and monitoring adherence
  • Participates in referral to community based care for patients
  • Complete evaluations based on client request for medical attention, emergency and non-emergency triage and physical/mental health rounds for safety and condition monitoring
  • Keep electronic health records up to date including progress notes and assessments of service.
  • Educating clients on their current medical diagnosis, needs and medical histories; educating clients on medication information and usages.
  • Alerts provider for any medication refills, needs of reconciliation or abnormalities and reactions occur.
  • Works in collaboration with counselors to ensure proper medication management and medication planning.
  • Works directly with Program Director on any medication errors and medication monitoring procedure
  • Works uniformly with Recovery Specialist on medication monitoring procedure
  • Ensure responsibilities regarding Duty to Warn and Mandated Reporting are fulfilled


For Familias Unidas Outpatient Program

Clinical Care Coordination

  • Coordinate patient care throughout the OBAT treatment episode.
  • Conduct nursing assessments and collect medical histories.
  • Monitor patient progress and treatment adherence.
  • Participate in multidisciplinary case reviews and treatment planning.
  • Facilitate referrals to higher or lower levels of care as clinically indicated.
  • Coordinate care with primary care providers, behavioral health providers, hospitals, detoxification programs, and specialty providers.

Medication-Assisted Treatment (MAT/MOUD) Support

  • Support prescribing providers in the management of medications for opioid use disorder (MOUD), including:
    • Buprenorphine products
    • Extended-release buprenorphine injections
    • Naltrexone (oral and injectable)
  • Assist with patient inductions and stabilization visits.
  • Monitor medication adherence and response to treatment.
  • Educate patients regarding medication use, side effects, storage, and overdose prevention.
  • Monitor for withdrawal symptoms, cravings, and relapse risk.

Nursing Services

  • Obtain vital signs and perform clinical screenings.
  • Conduct medication reconciliation.
  • Administer injectable medications as ordered.
  • Perform venipuncture and specimen collection.
  • Conduct CLIA-waived testing as authorized.
  • Monitor laboratory results and notify providers of abnormal findings.
  • Provide wound care and basic nursing interventions within the scope of practice.

Laboratory and Toxicology Monitoring

  • Coordinate urine drug screening protocols.
  • Collect and process laboratory specimens.
  • Track required laboratory testing, including:
    • CBC
    • CMP
    • Liver Function Tests
    • Hepatitis Screening
    • HIV Screening
    • Pregnancy Testing
    • Other provider-ordered laboratory studies
  • Ensure timely review and follow-up of laboratory results.


Qualifications

  • Massachusetts Licensed Registered Nurse
  • Bilingual English/Spanish skills are highly desirable due to the populations served by the organization
  • Knowledge of EHR systems
  • Ensure patient confidentiality in accordance with all federal, state, and local laws and regulations
  • Knowledge on best practices associated from the American Society of Addiction Medicine
  • Knowledge on Medication Assisted Treatment for substance use disorders
  • Knowledge of Substance Use Disorder
