Adult and Transitional Age Youth Intensive Case Management Programs

Program/Division Description

Our mental health programs provide comprehensive, recovery-oriented services to transitional age youth (TAY) and adults (ages 16-60) living with severe mental illness, many of whom also have co-occurring substance use disorders and complex physical health conditions. Our multidisciplinary team provides psychiatric medical care, case management, therapy, and crisis intervention services in both clinic and community settings, helping clients access housing, primary care, benefits, employment, substance use treatment, and other essential supports. This role is part of a collaborative team dedicated to promoting recovery, stability, and independence through person-centered, trauma-informed care. Community outreach is an integral part of the position, including providing medication support, conducting wellness checks, accompanying clients to medical appointments, and engaging individuals where they live to ensure continuity of care.

Job Description

The Registered Nurse (RN) is an integral member of a multidisciplinary behavioral health team, providing comprehensive nursing care to transitional age youth (TAY) and adults living with severe mental illness. Responsibilities include conducting psychiatric and health assessments; administering and monitoring psychiatric medications; assessing for medication side effects and changes in physical or mental health status; coordinating care with psychiatric providers, primary care providers, pharmacies, and other community partners; and facilitating referrals for additional medical and behavioral health services. The RN participates in crisis intervention, including supporting 5150 evaluations when appropriate, maintains accurate and timely clinical documentation in accordance with Medi-Cal and agency standards, manages an assigned client caseload, and provides consultation and education to non-medical staff on psychopharmacology and the medical aspects of client care. This position is based in our San Francisco clinic, with a combination of clinic-based services and community outreach, including medication support, wellness checks, and accompaniment to medical appointments as needed.

Specific Responsibilities

• Provides care coordination of complex medical and health needs of our clients including accessing and coordinating services.
• Utilizes a race equity framework and lens to address health and racial inequities in populations served by Felton.
• Assists with health promotion/risk reduction intervention and provides health education, screening, advocacy, and counseling for clients of all ages, including TAY and adults from marginalized populations.
• Utilizes harm reduction principles in education and interventions.
• Renders direct nursing services to vulnerable populations including TAY and adults with disabilities, special health care needs, clients with chronic illness, mental health, and/or substance use disorder; assists priority populations with access to medical care as needed; links/refers cases to appropriate human services, community, and safety net resources; maintains supervision of cases and keeps appropriate records of nursing and other services rendered.
• Administrative and clerical responsibilities, including support with scheduling, pharmacy communication, and maintaining the medication room according to DPH and Felton policies.
• Exercises medical responsibilities for psychiatric evaluation, medication assessment, and medication monitoring.
• Performs additional duties as assigned by supervisor or assignee.

Minimum Requirements and Qualifications

• Completion of training in an accredited Registered Nursing Program.
• Active Registered Nursing License from the CA Board of Registered Nursing.
• Experience working with SMI (severely mentally ill) populations in community mental health settings preferred.
• Demonstrated commitment to the welfare of individuals and families with mental and behavioral health issues.
• Knowledge of the information and techniques needed to diagnose and treatment human injuries, diseases, and psychosis. This includes symptoms, treatment alternatives, drug properties and interactions, and preventative healthcare measures.
• Complex problem solving, critical thinking, and social perceptiveness skills, i.e. being aware of other's reactions and understanding why they react as they do.
• Service orientation skills; actively looking for ways to help people.
• Demonstrated interest in ongoing learning and training.
• A team player with a welcoming attitude.
• Willingness to travel for outreach to clients in the community.

FLSA Status: Exempt

Salary Range: $140,000 - $160,000 Annually (DOE)
