Registered Nurse (RN) - Community-Based Clinical Review

Location: San Diego County, CA - Community-Based/Hybrid

Schedule: Full-Time, Monday-Friday - 8am-5pm PST

Service Area: Primarily Eastern San Diego County

Compensation: $110k-$125k with travel, cell phone & internet stipends

Role Summary

We are seeking an experienced Registered Nurse to support individuals with intellectual and developmental disabilities (IDD) who reside in Skilled Nursing Facilities (SNFs) and Intermediate Care Facilities (ICFs) throughout San Diego County. This is a unique community-based nursing role focused on conducting quarterly clinical reviews to help ensure each individual's current placement continues to appropriately meet their medical, skilled nursing, and support needs.

The RN will spend the majority of their time traveling to facilities and meeting directly with individuals, with flexibility to work from home for documentation and administrative responsibilities. This is an excellent opportunity for an RN who enjoys assessment, clinical review, care coordination, and working independently in the community rather than traditional bedside nursing.

Key Responsibilities

  • Conduct quarterly, face-to-face reviews of individuals residing in Skilled Nursing Facilities (SNFs) and Intermediate Care Facilities (ICFs).
  • Review medical records and evaluate changes in diagnoses, medications, hospitalizations, mobility, ADLs, medical equipment, supervision needs, referrals, and overall health status.
  • Assess skilled nursing needs
  • Evaluate whether an individual's current placement continues to appropriately meet their medical, nursing, health and safety, and support needs.
  • Identify changes in condition, potential risk areas, or concerns requiring additional records, referrals, provider input, or interdisciplinary review.
  • Prepare clear and concise written summaries of findings, recommendations, and appropriate next steps following each quarterly review.
  • Collaborate with Service Coordinators, clinical staff, facility personnel, healthcare providers, and interdisciplinary teams to support continuity of care and access to appropriate healthcare services.
Required Qualifications

  • Active and unrestricted Registered Nurse (RN) license in the State of California.
  • Graduate of an accredited school of nursing; Bachelor's Degree in Nursing (BSN) preferred.
  • Minimum of two years of nursing experience in a healthcare setting.
  • Strong clinical assessment, medical record review, documentation, and critical-thinking skills.
  • Ability to work independently while collaborating effectively with interdisciplinary teams and community providers.
  • Comfortable traveling regularly to SNFs and ICFs throughout San Diego County, initially with an emphasis on Eastern San Diego County.
  • Experience working with individuals with intellectual and developmental disabilities and/or in a consultative nursing role preferred.
